The ideal candidate will have a background in psychiatry, home health, or other related fields.
The QMHP will be responsible for providing comprehensive mental health services to clients in their homes and in the community.
This position requires excellent communication and problem-solving skills, as well as the ability to collaborate with other professionals.
Responsibilities:
Conduct mental health assessments and provide appropriate interventions.
Develop treatment plans and provide follow-up care.
Conduct home visits to assess the client's needs and provide support.
Provide crisis intervention services when needed.
Document all services provided in accordance with agency policy and procedures.
Provide referrals to community resources as needed.
Provide education and support to clients and their families.
Collaborate with other professionals to ensure the best possible outcomes for clients.
